Header and Claim Identification
Start with your practice letterhead, date, and payer appeals department address. In the first paragraph, identify the patient by name and member ID, the claim number, date of service, billed amount, and the CARC code from the denial. State clearly that you are filing a first-level appeal and request reprocessing of the claim. This opening paragraph gives the appeals reviewer everything they need to pull your claim file.
Denial Summary and Rebuttal
Restate the payer's denial reason in one sentence, then immediately counter it with your argument. Reference specific clinical findings, authorization numbers, or submission timestamps depending on the denial type. Use numbered points if you are addressing multiple issues on the same claim. Each point should connect a fact from your documentation to a reason the denial should be reversed.
The rebuttal section is the only part of the template that changes between denials. Match your argument to the CARC code — never submit a generic appeal letter for a specific denial reason.
Attachment List and Closing
List every document attached to the appeal by name: remittance advice, progress notes, authorization letter, operative report, or proof of timely filing. End with a direct request: reprocess the claim and issue payment within the payer's stated timeframe. Include your contact name, phone number, and email for the appeals reviewer to reach you with questions.

When a Template Is Not Enough
Templates give you structure, but payer-specific language, LCD references, and NCCI edit citations require customization for each denial. Writing 10 appeal letters per week at 45 minutes each costs your practice 7.5 hours of biller time — time that could be spent on clean claim submission instead.
