Authorization Denials

CARC 197 Authorization Denial Appeal

Authorization denials on a $15,000 surgical claim or a $900 imaging study are recoverable when you have the auth number on file. CARC 197 is one of the highest-overturn denial codes when documentation is attached.

Three Scenarios Behind CARC 197

First, no authorization was obtained before the service — common in urgent cases where precert was skipped. Second, authorization was obtained but the auth number was missing or incorrect on the claim. Third, authorization exists but the billed CPT code, date of service, or provider NPI falls outside the approved parameters. Each scenario requires a different appeal strategy and different supporting documents.

Appealing When You Have an Auth Number

Pull the authorization approval letter and compare the approved procedure codes, date range, and rendering provider against what was billed. If everything matches, your appeal is straightforward: attach the auth letter, highlight the auth number, and request reprocessing. If the auth number was omitted from the original claim, include a corrected claim or clearly state the auth number in your appeal letter with proof of prior approval.

Over 40% of CARC 197 denials occur when a valid authorization exists but was not transmitted on the claim. Check your auth records before assuming the denial is valid.

Requesting Retro Authorization

When no prior auth was obtained, call the payer's authorization department and request retroactive authorization. Document the call reference number, representative name, and outcome. Emergent services, payer system failures, and authorization obtained by phone but not recorded in the payer system are common grounds for retro auth approval. File your appeal simultaneously with the retro auth request.

Preventing Future CARC 197 Denials

Build an authorization verification step into your scheduling workflow. Match auth approval parameters to the scheduled CPT codes before the date of service. Enter auth numbers in the correct claim field and verify they transmit in your clearinghouse pre-submission edit report. One missed auth check costs more staff time on appeal than five minutes of front-end verification.
